Benefits for Active Employees

The Town of Millis offers a variety of voluntary benefits for its employees, including Health, Dental, Flexible Spending, and Life Insurance. Eligible new employees (those who are full-time or permanent part-time employees whose regular schedule is 20 or more hours per week) have 30 days to enroll in benefits. Failure to submit your election within the first 30 days of employment will result in exclusion from medical insurance and any voluntary benefits offered by the Town until open enrollment or in the case of a qualifying event.  The Town of Millis pays a percentage of the premium cost for the Health Insurance and Basic Life Coverage.

Health Insurance:

The Town of Millis currently offers health Insurance to eligible employees through the Group Insurance Commission (GIC). The Town pays 70% of the cost share for HMO, PPO-TYPE, and POS plans and 50% for PPO and Indemnity Plans.

New Hires - Newly hired benefited employees have up to 30 days from their date of hire to submit a completed a GIC enrollment form, along with applicable documentation, to the Human Resources Department. New employee coverage begins on the first day of the month following 60 calendar days from the first day of employment, or two calendar months, whichever comes first.

Qualifying Event - Employees who have a qualifying event can enroll in GIC health insurance, cancel coverage, and add or remove dependents. Documentation of the qualifying event and a completed change form must be sent to Human Resources to submit to the GIC within 60 days of the event. Spring Open Enrollment - Employees and retirees can makes changes to their plans during the open enrollment period. Open Enrollment typically runs during the month of April, and changes are effective on the following July 1st.

Dental Insurance:

The Town offers active employees dental insurance coverage through Altus Dental. The plans offered are group plans and are 100% employee paid. See below for links to plan information, rate sheets and enrollment forms.


Life Insurance:

The Town of Millis offers all active benefit eligible employees basic and voluntary life insurance plans.  These plans are insured by Boston Mutual Life Insurance Company.  The Basic Life plan offers $10,000 group term life insurance and accidental death and dismemberment coverage.  This policy is strictly a death benefit; there is no cash value associated with this plan.  The Town of Millis pays 50% of the premium for this policy. 

The Voluntary Life policy offers employees the opportunity to purchase up to $50,000 in additional life coverage. The Voluntary Life policy also has an option for Dependent Life coverage, Spouse for $5,000, and Children for $2,000. 

Upon the attainment of age 70, Life and Accidental Death & Dismemberment Insurance for both active and retired employees shall reduce to $5,000. All Life Insurance benefits for active employees and retirees, including dependent life will terminate upon the age of 75.
